We are offering a one day camp experience for campers. We will pair campers with counselors and they will rotate through activities within their group throughout the day. We will offer a "family lounge" and meals (see additional family form) for families that do not want to travel back and forth. Families are also welcome to participate with their camper if they choose. *

I understand my camper will be paired with a counselor and be in a group with others.

We are still being cautious and would like to attend as a family group instead of being paired with a counselor.

I expressly understand and agree that my family's participation in any programs outside my home presents known and inherent risks regarding any potential and/or actual infection of Covid-19 and/or any related illness, the result of which may be both serious and minor, including but not limited to, cough/fever, hospitalization/death. I understand that Chicago Central District (Everyone Cares Camp) and Dickson Valley have done their best to follow the most current health and safety guidance and that I am responsible to evaluate the risks that my family may face. by selecting YES below and engaging in camp activities, I have assumed the risks and am responsible for our participation. I understand that masks may be required during most activities during the day in accordance with the campground policy. We will comply with mask, sanitation and social distancing requirements. We will update the requirements in early July as this is an ever-changing event. *

I understand that upon arrival at camp, I will need to complete a health screening for anyone staying on the grounds. If you cannot pass the health screen you will be turned away. If you are required to quarantine and notify us by July 22, you can receive a refund. *

I understand EC Camp will be doing very limited medication monitoring and if my camper requires medication during camp, I will meet with the nurse at registration to check it in. *

Yes, we will be responsible for safely packing camper medications.

We will be using "elbow bumps" and "foot taps" instead of hugs and high fives. Please start practicing this with your camper right now! *
