Small Business Survey
Informed Consent Statement

You are being given the opportunity to volunteer to participate in a project conducted through John Brown University.

1. This survey is to gather data about small businesses and to discover areas in which the Gravette Public Library can assist.

2. You are completing this survey because you are either a small business owner, entrepreneur, work for a small business, or are thinking about starting your own or expanding.

3. This survey is composed of a series of questions rated on a Likert scale and open answer.

4. There are no known risks in completing this survey however there is a small time commitment to complete the survey.

5. Benefits of this survey will allow the Gravette Public Library to view what small businesses are needing/wanting in terms of knowledge through library programming.

6. The survey results will be anonymous and none of the data you provide will ever be linked to your identity.

7.  Persons under the age of 18 should not participate in this study.

8. Refusal to participate in this study will have no effect on any future services you may be entitled to from the University. You are free to withdraw from the study at any time without penalty.
