Please answer the following questions so that we may get an estimate for a guest count for our end of year Banquet on Wednesday April 27th 6pm.   This will help us determine which banquet room the venue will provide us as well as an accurate cost for ticket price.  The banquet will be at the Odyssey in Granada Hills on 4/27/2022.  Tickets will be between $65-$70 each depending on attendance count and will include Dinner, Dessert, Dancing, DJ and Awards ceremony.  Please let us know if you plan to attend and how many of your family members will be attending with you.  One questionnaire per family please.
