As a parent/guardian or a person with authority of the child/children referred to in this enrolment form, I declare that the information provided for the purpose of this enrolment is true and correct and that I undertake to immediately inform Divine Kids (the event organiser) in the event of any change to this information. I consent for Divine Kids to retain this information for future Divine Kids events like retreats, Bible Quests etc. I agree to collect the child/children, referred to in this form, if they become unwell at the service. In the event of an injury or a sudden medical emergency, I authorise the First Aid Officer present during this event to administer first aid medical treatment or seek emergency medical treatment for my child from a registered nurse or medical practitioner, hospital or ambulance service, if deemed necessary. I also understand and agree that the costs for transportation of my child in an ambulance service or any necessary expenses incurred by Divine Kids for such emergency medical treatment of my child will be covered or reimbursed by me. I acknowledge and accept the risks involved (for example: a fall when using the playground or the hall, an injury during playtimes or when conducting recreational activities during breaks) and I agree that Divine Kids will not be held liable for any loss of personal belongings and/or any personal injuries or damage caused during the event. *
