The Forest City Youth Film Festival is accepting music tracks that can be provided to student filmmakers in order for them to produce a short film for the Music Video category of the competition. Please fill out the following form to submit your music track for use in the festival.

I, the COMPOSER, who composed and own the sole and legal copyright to the submitted music track, agree to allow Forest City Youth Film Festival participants to use my work (song) in their music video productions at no charge for entry into the Music Video category of the Forest City Youth Film Festival. It is understood that any other use of this music track is to be negotiated separately. *

Permission Usage
The submitted song can only be used for productions entered into the Music Video category of the Forest City Youth Film Festival. If a student filmmaker's production becomes a Finalist in the festival, they agree to sign a release form with the Composer prior to the film being screened. The final film cannot be uploaded to the web or made public for any other purposes by the filmmaker without explicit written consent by the Composer. If the filmmaker requests to enter their Music Video production into other festivals, they must obtain explicit written consent from the Composer.
