Before we proceed, please carefully review the following instructions to ensure that your document request process runs smoothly:

1️⃣  Please be advised that it is your responsibility to submit the document request form at least four weeks in advance. Failure to do so may result in a delay of the document's production. Should such an issue arise, please contact the Program Director (Esther Rhi 604 882 3124), immediately to explore possible alternatives.

2️⃣ We would like to remind you that, in accordance with our Milaler policies, if you have served less than one year and your attendance falls below 90% per semester, your request for a volunteer document may be forfeited. We ask that you take this into consideration when making your request. If, however, you still wish to proceed, we must advise that your attendance and punctuality record may be reflected in the reference document produced. We appreciate your understanding of these policies and the importance of maintaining high standards of commitment and responsibility in our volunteer program.

3️⃣  Please note that each complete request form will only process one type of document. Should you wish to request multiple documents, please submit the corresponding number of forms.

4️⃣  We kindly ask that you provide all information to the best of your knowledge in the "Details" section. Your cooperation in this regard will greatly expedite the production process.

5️⃣  Lastly, please be aware that the contents of your document will reflect your performance and attitude during your time at Milal. Any instances of disrespect or tardiness may be included in the document, as we strive to provide honest and factual accounts.
