Thank you for completing this information form. Please be sure to read and share the following policies that we have in place for corporate projects. By initialing this form, you acknowledge and agree to abide by and follow Great Parks' guidelines.

1. Anyone participating in the project must sign a Great Parks waiver. The waiver will be emailed to the project coordinator no later than 2-weeks prior to project date, and we recommend filling them out prior to arriving on project day.

2. Volunteers must wear appropriate attire in order to participate. Any participants that do not have the proper attire may be unable to participate in the project due to safety concerns..

3. Great Parks will provide all training, tools and equipment needed to complete the project. Participants must follow all safety protocols and wear appropriate personal protection equipment (PPE) as deemed necessary by Great Parks. All tools and PPE will be provided by Great Parks.

4. A successful project depends on good communication. The project coordinator agrees to be the main liaison between Great Parks and the volunteer group and will communicate in a clear and timely manner. The project coordinator agrees to share information with the volunteer group related to the assigned project, including but not limited to start and end times, meeting location, what to wear and bring, maps and other pertinent project information.

5. Great Parks reserves the right to cancel a project or add, change or alter scope of work depending on but not limited to change in volunteer numbers, staffing levels, unexpected needs or repairs, available resources, changes in funding and the weather.

6. Great Parks will do its best to match your group with a project that meets your specifications. However, due to the large volume of requests received, this is not always possible. Opportunities are filled on a first-come, first-served basis and are dependent on available resources, park selection and group size. Signing this agreement does not guarantee your group a project.

7. I understand that participation in a project could be dangerous and that injury/illness (including communicable diseases such as influenza, MRSA, Coronavirus, etc) are possible.
