Please read the following thoroughly. You must agree to the following terms before proceeding with the application process. Refusal of these terms will result in the athlete not being eligible to participate in MPVC tryouts or become a MPVC member.

I understand that an updated athlete 2024 NERVA registration must be completed. **Note** This requirement should be fulfilled if the athlete participated in this past club season.

I understand that both the parent/guardian and the athlete must review and sign the Max Performance VC handbook and its policies. I understand that the athlete must have an updated USA Volleyball medical release form. Both can be located on the registration part of the website. I understand that refusing to complete this paperwork would result in the athlete being unable to become a member.

I understand that the athletes spot is not officially reserved for the 2024 Max Performance Beach Season until the $150 registration fee is paid.

I understand that any information on this registration form or on the USA Volleyball medical release form will be available to the athletes coaches, assistant coaches, other adult club personnel, and medical professionals as deemed necessary by Max Performance VC. I understand that by checking this box I am giving consent for this release of information.
