Informed Consent Statement
You are being invited to participate in the “Empirical Study for the Evaluation of Relational Schemas”.

The objective of this research is to investigate the understanding and usability of different relational schemas. The investigation is part of Gustavo Ludovico Guidoni's doctoral thesis,  under the guidance of Professor João Paulo A. Almeida, from the Graduate Program in Computer Science at Universidade Federal do Espírito Santo (UFES).

Your participation in this research is voluntary. If you agree to participate, the activities consist of answering a small number of questions about your (professional/academic) background and interpreting queries in specific relational schemas.

We estimate that the total time required to carry out the activities is less than 10 (ten) minutes (there are only 7 questions).

Your participation in the experiment will be as a volunteer, not receiving any privileges, whether financial or of any other nature. The data obtained during the research will be analyzed and consolidated for scientific dissemination purposes. Your responses will be recorded anonymously, not revealing your identity.

Those responsible for the research would like to thank you in advance for your participation and valuable contribution to the work.

(A previous version of this form was circulated in Portuguese. If you have responded earlier, we thank you for that -- please do not continue in this English version, as that would lead to duplication in your participation.)
I declare that I have read and understood the objectives of my participation in the experiment, as well as my rights, and I voluntarily agree to participate in this study. *
