Using SDOH Data to Support Patients with Complex Health Needs
LEARNING COLLABORATIVE DESCRIPTION:

Special and vulnerable populations often face additional barriers to care and are often compounded by social determinants. When screening for SDOH, health centers serving special and vulnerable populations will need to consider the unique needs and circumstances of the populations they serve. While screening for SDOH is a necessary first step in identifying these disparities, being able to analyze and understand the data is a crucial step in addressing SDOH.

While documentation of social determinants of health (SDOH) is critical, it's equally important for health centers to actively use this data to identify and address patient needs, drive organizational change, and improve the health of communities in which they serve. This learning collaborative will explore how to use SDOH data to support these different levels of change. Participants, in partnership with faculty, guest SMEs, and peers, will discuss strategies suited to their organizations. Participants will also have the opportunity to create a tool to demonstrate the value of SDOH data in their communities.

Participants will assist in developing their own impact matrix tool to demonstrate the value of SDOH screening and data for downstream, midstream and upstream level change. Participants will learn strategies for analyzing and utilizing data collected to support equitable care for their health center patients.

By the end of the LC, all participants will have the impact tool to guide SDOH screening efforts in their own programs and clinics. Participating sites may also have the opportunity to highlight applied practices with a wider audience in subsequent webinars, publications, and future iterations of this series.

ELIGIBILITY:  Prioritized applicants must be representatives of a Health Center Program grantee (or potential Health Center grantee)*

*Additional participants may be approved as space allows.

FEE: NO COST/ FREE

COMMITMENT: Accepted LC applicants are asked to attend a total of four learning sessions in March 2023.

EXPECTED OUTCOMES:
1)   Participants will understand the unique considerations of special and vulnerable populations when screening for Social Determinants of Health (SDOH).

2)   Participants will identify at least three (3) strategies to utilize SDOH screening data to effectively address SDOH of their patient populations.

3)   Participants will identify at least 1 impact strategy at the downstream, midstream, and upstream level to effectively promote equitable and high quality care for special and vulnerable populations.

4)   Participants will gain the tools to demonstrate the value of SDOH data.  

5)   Participants will learn from the cohort's experiences screening for SDOH and addressing social determinants and will share best practices for health centers.

Session Details

SESSION 1: Introduction & What to Expect (Wednesday, March 8th, 2:00 - 3:30 pm Eastern Standard Time)
By the end of session 1, participants will be able to:
1. Understand the value and impact of SDOH data
2. Understand the purpose of the impact grid
3. Understand the differences between Downstream, Midstream, and Upstream

SESSION 2: Care Plans (Wednesday, March 15th, 2:00-3:30 pm Eastern Standard Time)
By the end of session 2, participants will be able to:
1. Using a lens of cultural sensitivity and whole person care, discuss appropriate strategies SDOH data can be used for special and vulnerable populations
2. Propose a person-centered care plan using SDOH data intervention and work through key considerations required for success

SESSION 3: Health Center (Wednesday, March 22nd, 2:00-3:30 pm Eastern Standard Time)
By the end of session 3, participants will be able to:
1. Learn strategies to use SDOH data to make organizational change
2. Understand the impact SDOH data has on organizational policies & priorities

SESSION 4: Community/Policy (Wednesday, March 29th, 2:00-3:30 pm Eastern Standard Time)
By the end of session 4, participants will be able to:
1. Understand and list strategies for the impact SDOH data has on community and local policy change
2. Complete your impact matrix to meet identified goals for SDOH screening within special and vulnerable populations
