RiverNYC: Sunday Health Screening
To provide a safe environment and protect the vulnerable among us, we are requiring vaccination for all who are eligible and will conduct screening of anyone who are attending our services, which includes:
- Show proof of vaccination & a matching government issued ID (as currently required by NYC for indoor gathering)
- Wear masks at ALL TIMES indoors (as recommended by CDC), except those younger than 2 years old or medically at risk when wearing masks.
- Please only fill out this form within 24 hours prior to the Sunday Meetings (one form per group/family attending) and let us know if your condition changes after your submission.
If you answer YES to any of the required questions, we kindly ask you to stay at home.
