Kia ora! Before filling out this form, please remember that it will have to be filled out using your NX gmail account. Please also discuss with your parents which cellphone numbers and email addresses you are going to fill out. The contact details must be accurate so you will need a parent (or both) to be with you when completing any personal details. I will be using this form to make up contact groups for communication throughout 2021 if you are successful in your application. Some are compulsory so they have to be filled out. IT IS VERY IMPORTANT THAT YOU READ THE QUESTIONS VERY CAREFULLY, especially when completing what sports you have been involved in throughout 2020. (I do a check with Coaches and Teachers in Charge of each Sport.) All other information, CV/Resume instructions, important dates and deadlines are up on the School Website. I have created this Google Form so you can edit it after you have entered a response because there is still some sports to come in Term 4.
