Overdose Awareness Portrait Project Participant Interest Form
Healing Cortland is a group of community members and agencies working together to reduce opioid-related overdose deaths in Cortland County. A small group of us are working with a local documentary photographer Zack Bolton to capture and share stories related to opioid addiction and overdose to bring awareness to this issue. We are working on a portraits project to capture images and stories of those who have been impacted in some way and are willing to share their stories! 

You could be someone with lived experience of opioid use/ overdose, someone in recovery from Opioid Use Disorder,  family/friend, professional or impacted in some other way.
