Summer Reading Program Ticket Submission
Exchange your time spent reading for a prize entry by filling out the information below! This form can be used for any age group.

Children 9 and under will receive a small prize after reading for 8 hours.
Children between the ages of 10-18 earn 1 entry ticket for a prize every 30 minutes read.
Adults earn 1 entry ticket for each hour read.  
Ms. Emily will fill out your prize tickets for you and enter them into the prize options you choose below.

You can submit this form as often as you would like! No need to save up all your reading time and then submit.
