Thorncrest Friends Academy Release
In consideration of being permitted to enroll my child, Named in registration, in the children’s camp Managed by Thorncrest Friends Academy, also known as"Frances Shawera" (hereinafter referred to as "the Camp"), I, Parent filling out this form, hereby release Thorncrest Friends Academy, Frances Shawera, its directors, officers, employees, agents, volunteers, and any affiliated entities (hereinafter collectively referred to as "Released Parties") from any and all liabilities, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in the camp activities or while on the premises where the camp is being conducted.
I understand and acknowledge that participation in the Children’s camp involves inherent risks, including but not limited to, physical exertion, contact with other participants, and exposure to various outdoor and indoor activities. I am aware that despite all safety precautions taken by the Camp organizers, accidents and injuries may occur.
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney fees, that may arise from my child's participation in the camp activities or from any actions or negligence on my part or on the part of my child.
I authorize the Camp staff to secure any necessary medical treatment for my child in the event of any injury or illness that may occur during the camp program. I understand that the Camp organizers will make every effort to contact me or the emergency contact listed in the registration form before seeking medical treatment, but in case of emergency, I authorize them to act on my behalf.
I understand that Thorncrest Friends Academy is not responsible for any loss, theft, or damage to personal property brought to the camp by my child.
I agree that my child will comply with all rules, regulations, and instructions provided by the Camp staff for the safety and well-being of all participants.
This child care program is not licensed by the Government of Ontario. We strive to provide a safe and nurturing environment for your child, but it's important to acknowledge that our program operates independently and does not hold a government license. We encourage all parents to carefully review and consider the licensed child care options available in Ontario to make an informed decision that aligns with their family's needs and expectations. Your child's safety and well-being are our top priorities, and we welcome any questions or concerns you may have regarding our program.
I certify that I have read this Waiver and Liability Clause carefully and fully understand its contents. I acknowledge that I am voluntarily agreeing to its terms and conditions.
