As you may know, there are many inherent risks of personal injury surrounding any participation in any contact sport. Football is a collision sport. With as many changes to rules, improved conditioning programs and protective equipment, even better medical procedures for treating injuries, all of this cannot eliminate RISK. Rather, these efforts only reduce the chances of injury. Our players will adhere to all safety rules designed specifically for them and their health and safety. We expect our players to report all physical injuries to a coach, who, in turn, will report the injury to the athletic trainer and head coach. Players are required to check their equipment daily before they leave practice for the day and will report any damage to their equipment. Damaged equipment will be fixed or replaced before the player participates. Players will be expected to participate in our conditioning program for this reason. RISK is substantially reduced when a player is physically in shape and has prepared himself for the rigorous demands required by this sport.

READ BEFORE SIGNING

In consideration of being allowed to participate in any way during the Bonneville High School eight-day padded camp for 2020 (June 17-26), as declared by the Head Coach, Athletic Director, and Principal to the UHSAA, the undersigned acknowledges, appreciates and agrees that:
1. The risk of injury from the activities involved in this camp is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest coach immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless, the Board of Education of the Weber School District, its officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASES”), with respect to any and all injury, disability, death, or loss or damage to person or property, during practice or while en route to or from practices/events; and,
5. I hereby give consent for my son to participate in the eight-day padded football program; and,
6. I have adequate insurance coverage under a policy held for the family.

I, have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights, by signing it, and sign it freely and voluntarily without any inducement. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above, of all the Releases, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releases from any and all liabilities incidents to my minor child’s involvement or participation in these programs as provided above.
