2022-23 CWMS Schedule Change Request Form
Your schedule is based on the courses you requested in Spring 2022. Changes requested via phone or email will not be honored. Any schedule change requests must be submitted via this form:
-The counselors do not take requests via phone or email.
-You must submit one request per form; you may submit multiple forms.
-Requests for teacher and/or period changes cannot be accommodated.
Schedule changes made during the first week of school (August 9th – 12th) will be for the following reasons:
-A required course is missing (e.g.: English is not on student’s schedule)
-There is a hole in the student’s schedule (e.g.: Student does not have a course in period 2)
-Incorrect level placement (e.g.: Student is enrolled in Spanish 1A, but should be in Spanish 1B)
-Dropping an elective to go from a 7-period to a 6-period day
After all the above courses are changed we may consider preference changes (e.g.: I would like to take Foods instead of Art )
Request changes will not be accepted later than Friday, August 12th. If changes are made you will receive notification from your counselor no later than Wednesday, August 17th.
