WRU Fit Fed Fun Camps Registration Form (Cardiff Region)

This form is for parents, legal guardians and/or establishments to register that attendance of children to the WRU’s fit fed fun camps which will take place during the 2023 summer holiday period (exact dates TBC) (the “WRU Fit Fed Fun Camps 2023”), in collaboration with the Cardiff Rugby Foundation.

 

Please note that this is a registration form and the eligibility of any applicant to attend the Cardiff Rugby Foundation WRU Fit Fed Fun Camps 2023 will be determined by the WRU in its sole discretion.

 

Data Protection

 

The WRU acknowledges and respects the privacy of individuals. The information that is being requested in this form is required to allow the WRU to (i) process your expression of interest in relation to the WRU Fit Fed Fun Camps 2023 and the corresponding application process for attendance at the WRU Fit Fed Fun Camps 2023 and (ii) determine pre-existing conditions which may affect any participant’s ability to participate in the WRU Fit Fed Fun Camps 2023.

The intended recipients of the data requested within this form are the WRU and its authorised staff, agents and personnel, to allow the WRU to exercise its responsibilities towards you and/our your nominated child in relation to your and/or your nominated child’s attendance at the WRU Fit Fed Fun Camps. In order to process your expression of interest in attending the WRU Fit Fed Fun Camps 2023, the WRU may also contact the person who completes this form via post, telephone or e-mail regarding the application and/or otherwise in connection with the WRU Fit Fed Fun Camps 2023.

 

If your application to attend the WRU Fit Fed Fun Camps 2023 is successful, the WRU will also need to share certain information for attendees of relevant camps – such as dietary or accessibility requirements – with authorised service providers (Cardiff Rugby Foundation) of the WRU for the purpose of those service providers fulfilling and delivering certain services at the WRU Fit Fed Fun Camps 2023 on behalf of the WRU.

 

Please note that by completing and submitting this form to the WRU, you are consenting to the WRU processing and/or sharing the personal and sensitive data contained within this form in this way and for the identified purposes. If you do not consent to the same, the WRU will not be able to process your expression of interest in attending the Cardiff Rugby Foundation WRU Fit Fed Fun Camps 2023.

 

If your application to attend the Cardiff Rugby Foundation WRU Fit Fed Fun Camps 2023 is successful, please note that we may also need to share the information contained in this form with the University of Gloucestershire, who are working with the WRU to produce a research study in relation to reviewing the experience of participants who attend the WRU Fit Fed Fun camps.

Consent to disclosing relevant participation data to the University of Gloucestershire

 

I consent to the WRU disclosing relevant data relating to my child to the University of Gloucestershire, for the sole purpose of enabling the University of Gloucestershire to work with the WRU to produce a research study in relation to reviewing the experience of participants who attend the 'Fit Fed Fun Camps'.

 

I consent to the above.

In respect of my child’s potential participation in the WRU Fit Fed Fun Camps 2023, I agree and acknowledge that:

(a)  the data provided in this registration form is true to the best of my knowledge and belief;

(b)  the WRU Fit Fed Fun Camps 2023 will involve participation in a number of physical exercises, including but not limited to tag rugby and rugby related games / activities, which my child is fit and able to participate in if they are eligible to attend a camp;

(c)  my child will abide by all instructions given to them by the WRU, it’s employees, agents and instructors in relation to the exercises in which they will participate if they attend any camp; and

(d)  if my child attends and participates in the WRU Fit Fed Fun Camps 2023, any such attendance and/or participation is at their own risk and I (on behalf of my child) waive any and all claims against the WRU and/or its employees; agents or servants in relation thereto. Nothing in this disclaimer shall exclude or restrict the liability of the WRU for death or personal injury resulting from the negligence of the WRU and/or its employees; agents or servants while acting in the course of their employment/engagement or any other liability which cannot be excluded or limited by law.
