This form is required to apply for permission to participate in competition hosted locally or outside Malaysia. You are required to submit this form 1 month (if within Malaysia) or 2 months prior (if outside Malaysia) to the competition. The Office of Deputy Dean for Student Affairs (TDHEP) will review the application and revert within 14 working days.
