All responses will be kept confidential under the Data Privacy Act (RA 10173). Rest assured that they will be used only for the purpose of this study and will be viewed only by the primary researcher, thesis adviser, and the consulted statistician. For open-ended answers, respondents will be assigned code names to ensure anonymity. You may also withdraw anytime if you feel uncomortable in ansering the survey.
