Request for Academic Plan Change/2nd Semester
As mentioned prior to school starting, we ask that families select a learning option for a semester at a time. Should you wish to request a change to your student's academic plan for second semester, please complete the form below by December 4.

If we do not receive a form from your family by December 4, we will assume your student will continue with the same academic plan they are currently on. If you indicate a request to choose a different academic option by completing this form, the school will follow-up after December 4 with more details.

Please complete one form for each student who wants to request a change in their academic choice for second semester.
