This information will be used by the Eco-Reps to assess and award residents with a Green Room Certificate if residents' practices meet Green requirements. After submitting this form, an Eco-Rep will evaluate your responses and award you with a Green Room Certificate if earned.
Those who successfully earn a Green Room Certificate will be entered into a drawing to win a sustainable prize!(Individuals must complete their own survey to be entered into the drawing.)
