圆点心宁中心为迎接来临新的一年，于2020年的首日又再特别为现代繁忙的都市人举办《四念处》一日修，希望大家踊跃参与，共沾法益！

报到：当天早上九点请于柜台签到
穿着：白色圆领T-shirt及宽松深色长裤，以简朴素雅为妥
自备：请携带水壶/保温瓶，披肩/毛毯（视个人所需）

1) 在提交报名表格后的7天之内，请将您的支持费（会员 $50 | 非会员 $60) 以现金或支票的方式交给圆点心宁中心柜台，或通过ATM机／网银汇到以下账户:

4) 只提交表格而未缴费的报名者，将无法获得课程名额。

退款详情：

Fees paid before commencement of the course are refundable upon receiving student's written notice of withdrawal, subject to the following:

- 50% refund if written notice is received at least 30 days before the course commencement date.
- 20% refund if written notice is received at least 15 days before the course commencement date.
- 0% refund if written notice is received on or after the course commencement date.

Data collected will be treated as confidential and used by the centre only for official purposes. Once accepted into the relevant course(s)/programs, I may from time to time participate in events organized by DCGC whereby my image may be captured in photographs and video-recordings. By submitting this form, I hereby consent to the use of the data for the purposes indicated, and photographs or videos containing my image on DCGC's website, social media or publications for publicity purposes.
