Bear Creek HS Parent/Guardian Volunteer Form
If you would like to be a volunteer for Bear Creek High School, please complete this form.

After completing this form, the next step in the process is to fill out the Lodi USD Volunteer Request Form.  Once that form is submitted and approved, Lodi USD Personnel Department will contact you with information on how to get your fingerprints completed. 


Further, if you’re interested in driving on Co-Curricular, or Extra-Curricular trips, you will also need to complete the Volunteer Driver Application Form.
