This form is for families and professionals to request a space in one of our groups.

At the moment we are at capacity but are working to expand our provision from September 2024.

Spaces rarely come up at our Forest School. Children don't want to leave us and some have been attending for 10 years!

This form is to apply for a possible space at a Forest School group with Mud Pie Explorers CIC. Filling this form does not guarantee a space.

● Those that require personal care/ support with toileting

Sadly, not everyone who asks for a place will get one.
