Hello parents and guardians,

I am collaborating with Mr. C (music teacher from River Mill) to create a Halloween Rap music video featuring your students! The music video will feature every class playing instruments/singing along to the awesome song Mr. C wrote. Once the music video is edited and finished, it will be shared with families from RME and CRE. We would also like to post it on Mr. C's public, educational YouTube page called "Music with Mr. C". If you are comfortable having your student(s) seen in the music video that will be publically shared on YouTube, please select "Yes."

If you are NOT comfortable having your student(s) seen in the music video, please select "No." Your student(s) will still participate with their class while filming for the music video, but they will not be seen. Only students whose parents select "Yes" will have their student(s) visible in the music video.

Please fill this form out before 10/15 or it is assumed your student(s) is NOT released to be in the public music video.
