The undersigned hereby releases the City of Ottawa,  and Dunrobin Community Associations and its successors and assigns from any and all claims, damages, actions or causes of action which may arise by reason of the participation of myself  in the above program sponsored by the above community association. I attest and verify that I have full knowledge of the risks involved in participating and that I am physically fit and able to participate in said events. You also consent for your photo to be used on the community association websites.
