Note: Failure to notify the McCulloch Center for Global Initiatives of any changes of your mailing address and employer information within 10 days of the change will result in your falling out of legal immigration status. Failure to provide employer information will result in termination in the SEVIS system 90 days after the start date on your EAD card. IF YOU HAVE USED THE SEVP PORTAL TO UPDATE YOUR INFORMATION, YOU DO NOT NEED TO COMPLETE THIS FORM AS YOUR INFORMATION HAS BEEN UPDATED IN SEVIS.
