As we prepare for a variety of "back to school" scenarios this fall, Westside Community Schools recognizes that some students may present with compromised immune systems or other health issues that pose a risk for their in-person return to school.

Please complete the survey for each child in your family.  

Your response to this survey will help us to 1) identify which and how many students may be at-risk or electing not to return in the fall, and 2) alert us to the potential need for a child(ren) in your family to receive an individualized health plan or accommodations while on campus.  

Similar to all health information submitted by parent(s)/guardian(s) to the district, this information is FERPA protected and will be kept confidential. Only district administrators and designated building staff will be made aware of survey responses.
