Thank you for requesting a device from our school for use in Scenario 3 suspension of classes/school closure.

We are committed to providing quality instruction in an online learning context, if that is required of us during this school year.

Please note, the device you are borrowing is important to us. We have a limited number at school to serve our entire school population. This device is expensive and has been purchased with funds raised by our school community. We will need this device to continue the excellent work in technology that we are engaged in.

We ask that you, the parent/guardian, agree to the conditions of this loan. Please read all sections of this user agreement, and respond to the questions. We need to ensure that you understand your responsibilities with regards to this agreement to receive this device.
