I authorize my learner to participate in Play Club at TVS. It is understood that “Play Club” refers to those unstructured activities taking place after school from 3:30pm-5:00pm. I understand that a TVS staff member will be present solely in a “lifeguard role” to monitor for safety. I agree to be prompt in picking up my child at 5pm. A $10 late pick up fee will be charged.

By completing this form, I agree to release The Village School from any and all claims, demands, or causes of action, which are connected with my child's participation Play Club or the use of the equipment and facilities.
