You must be a resident of District 2 to apply to East Side Middle School.  

PLEASE NOTE: This form is NOT to be used for students currently enrolled at another District 2 middle school.

This form is ONLY for applicants who have recently moved into District 2 (you will need to provide proof of residency to the Family Welcome Center) OR for applicants currently living in District 2 that are enrolled in a private (non-DOE) school or charter school. All new families must begin by contacting the NYC DOE Family Welcome Center at 333FWC@schools.nyc.gov 

*Families interested in 7th or 8th grade placement for next school year (September 2022):

This is an internal East Side Middle School form; it is not connected to the Department of Education admissions process.

Please complete a separate form for EACH child seeking a seat.  All placements are ultimately made based on seat availability.
