Maple Grove Senior High School Hall of Fame Nomination Form
This award is designated for alumni, staff, coaches, advisors, or community members who have made significant contributions to, and/or achievements in Maple Grove Crimson Activities or Fine Arts.

Nominees for this award must be 1) alumni who graduated from Maple Grove at least five years ago (Class of 2018 or earlier), 2) retired staff members/coaches/advisors, or 3) community members who have had a distinguished influence on the Maple Grove Senior High School Activities Department.

Up to five individuals may be inducted each year; new inductees will be honored at halftime of the Homecoming Football game on Friday, October 6.
