The criteria will be:

- Outstanding initiative and innovation demonstrated in their role.

- Outstanding impact and benefit to the community or organization through the volunteer’s effort.

- Outstanding commitment to the organization over a significant period of time with measurable and exceptional outcomes.

- Outstanding inspirational role model for the organization and its members able to be demonstrated with tangible examples that can be detailed.

- Outstanding achievements for the organization over time.

Please complete the selection criteria questions numbered from 1 to 5. Each of the selection criteria should be addressed and should be limited to a combined total of a maximum of 750 words.

When answering the questions, it is important to note the following:

Your personal information is being collected by Volunteer Sunshine Coast to allow a panel of judges to consider nominations for National Volunteer Awards and select winners for the above categories.

Your personal information will only be used by the panel to select award recipients and will not be used or disclosed for any other purposes.

By submitting this form, the nominator grants Volunteering Sunshine Coast the right to share this information publicly.

The successful award winners and finalist’s photo and profile from each category will be showcased on our website along with our official Principal, Gold and Silver Sponsors. Media permission will be required to ensure all finalists and award winners are highly profiled in all of our Sunshine Coast Region.

All nominations must be received by Volunteering Sunshine Coast by close of business on 23rd April. Finalists will be announced on April 29th 2021. The presentation ceremony for finalists, members of organizations and Award recipients will be held at a venue to be advised on Thursday, May 22nd. Invitation only function.
