Only fill out this form if you plan to join the Constitution and Annual Meeting via zoom. This is information we will use to set up the ballots so you are able to vote. To ensure the vote is valid it needs to be anonymous so by registering voters this way we will not see names when votes come in. Those joining in person or by phone will vote a different way and do not need to fill out this form. If you have multiple people joining the meeting you will need to register EACH VOTER with a different email address to vote even if you share a zoom screen. If you only have one email address please contact the church office and we will give you instructions on how to vote.
