The By Degrees Foundation is seeking nominations for the first annual Middleton Award for Children’s Advocacy. The Middleton Award recognizes individuals who have demonstrated commitment and dedication to children in central Iowa. This award was created to recognize the community effort needed to ensure more children have access to resources that help them thrive.
The award will be presented to an individual that, through their work, volunteer efforts, advocacy, public policy, or philanthropy, has made a significant positive impact in the lives of children in central Iowa. 
In addition to the following information, you may submit up to two pages of additional documentation in support of the nomination. Additional materials may include letters of support, photos, additional information on their work, etc.
