This is to be filled out by those who are requesting money from the Emily Waseman Foundation. The Emily Waseman Foundations main goal is to support youth in our community of all ages. Just as Emily would have done, we are here to provide things for youth who may not be able to be able to afford on their own. All information is made private and confidential and may only be shared with the Emily Waseman Foundation Board Members without consent.
