EFBA CARPOOL REQUEST FORM - 2024

Dear EFBA families, please complete this form if your child(ren) will be attending (or you are considering enrolling in) our summer camps or afterschool programs for 2023 and you would like to arrange a carpool with other families for drop-off and pick-up. Responses to this form will be used to create a directory map showing families in your area who wish to participate in a carpool (exact addresses will not be collected or shown). If you do not wish for your name or contact information to be listed in the directory, please indicate your choice on the form, your place on the map will appear anonymously as "local EFBA family". This directory is a resource that EFBA makes available to the families to help coordination between them. EFBA disclaims all liability with respect to any carpooling activity and you engage in carpooling and sharing information with other families at your own risk.
