This form must be completed for any student who would like to try-out for the boys basketball teams at Cedarbrook Middle School for the 2022-2023 season. The information entered into this form will be provided to the Cedarbrook co-athletic directors and coaches.

Please take note that a completed PIAA Sports Physical must be turned in to the Cedarbrook co-Athletic Directors before any student may try-out for any team. Blank copies of the forms are available for download on the Cedarbrook Athletics website or can be picked up at the main office. Students who turned in a PIAA Physical for fall sports do not need to resubmit the physical.
