TURKEY CUP WAIVER
Release and Liability Waiver – Media Usage Permission
(must be signed by parent or guardian if participant is a minor)
I, for myself or as the parent or legal guardian of (minor child if applicable), and as
lawful consideration for myself or my minor child being permitted to participate in the TURKEY CUP (“TC”), I agree that neither I nor my minor child will make a claim against, sue, attach the property of, or prosecute FC Frederick, Inc., and their officers, agents, and employees for damages for death, personal injury, or property damage which either I or my minor child may sustain as a result of participation in the TC. This release is intended to discharge in advance FC Frederick, Inc., and their officers, agents, and employees from and against any and all liability, including for negligent actions, arising out of or connected in any way with my or my minor child’s participation in the TC.

I FURTHER UNDERSTAND THAT SOCCER INVOLVES PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH ACTIVITY, AND THAT PARTICIPANTS OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF, KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE THAT I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS FC FREDERICK, INC., AND THEIR OFFICERS, AGENTS, AND EMPLOYEES WHO (THROUGH NEGLIGENCE OR CARELESSNESS) MIGHT OTHERWISE BE LIABLE TO ME, MY MINOR CHILD (OR OUR HEIRS OR ASSIGNS) FOR DAMAGES.

I attest that I am eighteen (18) years old or older, and that I and my child (if a participant) are physically fit and have no known medical conditions which prohibit participation in this sport. My child and I agree to follow all rules and guidelines regulating the conduct of the TC.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR MYSELF AND MY CHILD AND A CONTRACT BETWEEN MYSELF, MY CHILD AND FC FREDERICK, INC., AND THEIR OFFICERS, AGENTS, AND EMPLOYEES, AND I HAVE SIGNED IT OF MY OWN FREE WILL.

Additionally, I hereby grant permission to FC FREDERICK, INC. to use my or my child’s film/video/photo image for display on their websites and/or to use my film/video/photo image in any digital medium or form, including educational, instructional, or documentary films, videos, movies, DVDs, CD-ROMs, or printed materials.

By completing any or all of the "Player Name" lines below you are certifying that said person, or guardian of a minor, has read and fully understands the Turkey Cup Waiver 2021
