I do hereby consent and agree that CIOFS-FP, its employees, or agents have the right to take photographs, videotape, or digital recordings of me throughout my participation in the Erasmus+ KA3 project Eduwork.Net (Project Ref: 609096-EPP-1-2019-1-IT-EPPKA3-VET-NETPAR) and to use these in any and all media, now or hereafter known. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

I do hereby release to CIOFS-FP, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.

I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.

I also understand that CIOFS-FP is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result.

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.

How We Use Your Personal Information: The personal information you provide will only be used for the purposed of the above stated project by CIOFS-FP and kept on file for the duration stated by the funding providers.  The information will be is passed to the relevant project partners and the relevant funding agencies.  Further information about use of and access to your personal data, and details of organisations with whom we regularly share data are available at: http://www.ciofs-fp.org . You have a right to see the information which is being held about you. Information may also be shared with your agreement with other organisations providing learning, support and employment opportunities.
