Please complete the following form to indicate your intention to undertake a sacrament for 2023. This will help the parish to understand how many candidates will be participating in programs. The information collected also provides a means of communication between parish and families.

Please note: this form does not ensure enrolment - this takes place next year when the program is opened.

Reconciliation (requirements: Baptised in the Catholic church. Normally offered to Grade 3)
First Communion (requirements: Baptised & Reconciliation. Normally offered to Grade 4)
Confirmation (requirements: Baptised, Reconciliation & First Holy Communion. Normally offered to Grade 6)

However, there may be families who are requesting catch-ups for a variety of reasons.
