It is vital that we do all we can to ensure that children and staff are well and able to attend school.  Please complete this form on the FIRST DAY OF YOUR CHILD'S ABSENCE FROM SCHOOL. If your child is not back at school for the following Monday, please complete another form on a weekly basis, until your child returns. Thank you.

Please complete Section 2 if your child is unwell.
