Children, Youth and Family Faith Formation Registration

Thank you for your interest in participating in our faith formation classes at The Lutheran Church of The Good Shepherd. This year we will be offering Sunday School for children Preschool to 5th grade. Faith Explorers will be offered for youth in grades 6 to 12.

To support our programming process, please submit this registration form for your child prior to your first attendance. A separate form will needed for each child in your family attending our program. Registration forms will also be available in print at LCGS if needed.
