O. Henry Athletics Survey (7th/8th only)
Parents and Guardians, we are working on figuring out what sports will look like at O. Henry for Fall 2020.  Austin has recently moved in to Stage 3 Covid guidelines which recommend no social gatherings of more than 10 people.  Your coaches want to coach when it is safe to do so… in preparation for resuming sports, we have a few questions we’d like for you to answer. Thank you for your time.

I understand schools will not compete against each other until we reach Stage 2 Covid-19 Risk Based Guidelines. *
What sports do you expect your student to try out for this Fall given current conditions? *
Required
As we plan for after school participation, what is your zip code? *
Could you provide transportation for your child to OHMS several evenings a week? There will be no morning practice. There will be no district provided transportation to school. (Parents are not allowed to enter campus buildings or fields due to safety protocols). *
Will you be able to submit all the proper documentation (physicals, medical history, online Rank One forms, waivers, etc)? *
Will your child be able to aid in screening through the AISD app prior to each practice (personal cell phone)? *
I understand that locker rooms will not be available and students will be expected to bring their own water, be dressed out and bring all equipment home each day. *
I understand if we go back to Stage 4, sports will be virtual *
I understand if someone on the team or the coaches test positive for Covid-19, my child will have to quarantine for 14 days. *
I understand there will be no spectators allowed for indoor sports during Stage 2. *
I understand football and volleyball players will be required to wear a mask and mouthpiece underneath their helmet (football) at all times during tryouts/practices/games. *
