* Complete your child's registration forms!

Please MAKE SURE you bring the following documents:
- Offer Letter
- Child's Original Birth Certificate or Passport
- Parent/Guardian Identification (i.e. Driver's License, Passport)
- TWO Proof of Address (i.e. utility bill, lease)
- Proof of Immunization

If Applicable please also bring:
- IEP or Related Service Documents
- 504 Accommodations
- Guardianship Paperwork

Your child MUST be present for registration, unless your child is enrolled in our Pre-Kindergarten program.
