Hello Parents: You are receiving this form to register your child, grandchild or godchild for Sunday school this 2020-2021 year.  Please fill out the form below so that I know your child plans to attend or use one of our Sunday school formats! Note, if you have more than one child, I will need you to fill out the form for each child.  Don't worry...it is a very short form-- it should take only a couple minutes per child.
