MEDICAL CONSENT

In case of medical emergency, I understand every effort will be made to contact the parents/guardians of the child(ren). In the event that I cannot be reached, I hearby give permission to the medical examiner selected by the Awana staff to hospitalize, to secure proper treatment for, order injection, anesthesia, or surgery for my child(ren) as named on this form.

ENROLLMENT

I hearby enroll and give permission for my child to participate in the planned activities of Cambridge Awana Club held at Willerup Church. I understand that I am responsible for transportation to and from Awana Club. I also agree to hold blameless the Awana Clubs, Awana volunteers, Willerup Church and staff for all activities.
