Please provide the following information for both of your student-athletes.  DUE BY February 28, 2023

Total number of points is weighted in the following manner:

1. Athletic Involvement 50 points
2. Scholarship 20 points
3. Leadership 10 points
4. Deportment 10 points
5. School/community              10 points
        involvement

Note:  Candidates must be senior students.  Transfer students may be considered.  Previous records should be considered.
