We are really excited to get to a place where we are looking to print our Appointments and Transitions resources! These are the product of over a year of hard work from the LUNA team, young people with and without long term health conditions, and  healthcare professionals. They are designed as a companion for young people as they go through the healthcare system - supporting them with getting the most out of appointments, managing emergencies, becoming independent, and navigating a range of different transitions that come with growing up! Each resource contains interactive pages, interviews with young people and health professionals, top tips, communication strategies and lots more! The set of two come in a button folder for people to put appointment letters in and other information given at appointments with a checklist on front with some ideas for what to include.
